Cagrilintide is a long-acting amylin receptor agonist, engineered with the same fatty-acid-based half-life extension technology used in semaglutide, allowing once-weekly dosing in clinical research. Amylin is a hormone co-secreted with insulin from pancreatic beta cells that acts on distinct amylin receptors in the area postrema and hypothalamus to slow gastric emptying and promote satiety through a mechanism separate from — but complementary to — the GLP-1 pathway. Because amylin and GLP-1 signaling act on different receptor systems that both converge on appetite regulation, cagrilintide has been studied extensively in combination with semaglutide (as the investigational combination CagriSema), with published trial data reporting greater weight-reduction outcomes than semaglutide alone.
